1) Fill out the 2012 CYSA-South Player Registration Form

2) Show Proof of Age: 

CYSA-S Rules & Regulations 1.2.1. Proof of age shall consist of a birth certificate, a Uniformed Services Identification and Privilege Card (DD Form 1173) issued by the Uniformed Services of the United States, a birth registration issued by an appropriate government agency or board of health records, a passport, an alien registration card issued by the United States Government, a certificate issued by the Immigration and Naturalization Service attesting to age, a current driver's license, or a certification of a United States citizen born abroad issued by the appropriate government agency. Hospital, baptismal or religious certificates will not be accepted. (USYSA Rule 204) (Amended May 20, 2000)

CYSA-S Rules & Regulations 1.2.2. Reproductions or photocopies of documents that have not been officially certified by the issuing government agency will not be accepted.

3) Payment by check or money order only. Check made out to "Mesa Soccer". Please write a separate check for each child registered. No credit cards accepted.

4) MAIL IN REGISTRATIONS:Please fill in Cal south medical release form and submit it together with a copy of birth certificate or passport and full payment, to:

5) $25 Late fee for all registrations postmarked or received after February 22, 2012.  Player will be allocated to a team on a "space available" basis only.

6) Your coach should call you two weeks prior to the start of the season. If you do not hear from your child's coach, please call (858) 558-MESA [6372]. 

If your player is not quite ready for league games or they just want to brush up on additional skills, check out our Little Nott's program for 4-10 year olds.

New and Returning Mesa Recreation Volunteer Coaches

Mesa Recreational Coaches are always in demand.  Mesa provides monthly coaching sessions and access to Nott's Forest competitive coaches for additional support. 

If you are a returning or new coach, please submit Cal South Program Administrator Form with your child's registration. 

Refunds

1) Refunds are not granted after the last registration date except with approval of the finance committee.

2) There will be a $25 non refundable processing fee taken from a refund to cover CYSA and administrative expenses.

3) Refunds are not given for requests not accommodated. Requests are considered but not guaranteed.

4) Once approved on case by case basis, refund processing will take 4-8 weeks.  A check will be mailed to the registrant's home address.
